The month you turn 65 starts a clock. Medicare gives you seven months, the three months before your birthday, your birthday month, and the three months after, to sign up for Part A and Part B.

Timeline
Your Cincinnati Medicare countdown
3–6 months before your birthday
Figure out whether Part B should start now or wait because of active employer coverage. Pull together your list of doctors and prescriptions.
3 months before
File for Part A and Part B through Social Security if benefits haven't started yet. Already collecting Social Security? Your card shows up on its own.
2 months before
Pick your lane: Original Medicare paired with Medigap and Part D, or a Medicare Advantage plan. Check networks against your Cincinnati providers and your drug list.
1 month before
Send in the paperwork so your card lands before your start date. Make sure nothing from your old coverage lapses in the gap.
After your birthday
Your six-month Medigap shopping window starts the day Part B kicks in. Use it. It's the one stretch when your health history doesn't count against you.
Still working?
A job changes your timeline
Still working, or is your spouse, at a company with 20 or more employees? You can usually push back Part B with no penalty and stay on the group plan. Most people still take Part A since it's typically free, unless you're funding an HSA. Enrolling in any part of Medicare stops HSA contributions cold.
Once that job ends, you get eight months to sign up for Part B under a Special Enrollment Period. COBRA and retiree coverage don't stretch that clock. Getting this wrong is the costliest misstep we see, and you can run the numbers with our Part B penalty calculator.
Small employers work differently
Under 20 employees, Medicare typically becomes your primary coverage at 65, and putting off Part B can leave gaps. Check with HR at your company, whether it's in downtown Cincinnati or out along the Blue Ash business corridor, before you decide.

- Original Medicare + Medigap + Part D: any doctor who accepts Medicare, a higher monthly bill, costs you can predict
- Medicare Advantage: lower monthly bill, a defined network, copays per visit, extras like dental and vision
